School Forms This page makes available the various forms that are needed by prospective applicants and families that are currently enrolled at the Friendship Children's Center.
Student Activities Permission Form (PDF format)
This form describes the planned activities for the coming year. Parents who wish for their children to participate in these activities must sign this permission form and return it, along with the appropriate activity fee to the school no later than September 1.
Annual Student Activity Fee (PDF format)
This form lists the annual student activity fees for each classroom.
Enrollment Application (PDF format)
Families who submit an enrollment application and pay the appropriate fees will be placed on the school's waitlist. They will also be told of the approximate waiting time until a space becomes available in the appropriate classroom. There is no guarantee that a space will become available at the time services are needed or when the space was anticipated to be available. Siblings of children currently enrolled in the Friendship Children's Center are moved to the top of the waitlist, followed by children of school employees. Wait-listed parents are encouraged to inform the Director of any changes in the status of their application or request for care.
When a space becomes available in a classroom, current part-time families seeking full-time care will be given the right of first refusal. If the space is still available, the Director will contact the next family on the waitlist to inform them of the opening. Families seeking full-time care will be given preference whether or not the available space is full-time or part-time. Priority consideration is given to individuals who live or work in the Chevy Chase DC area to ensure that the Friendship Children's Center remains a community school.
Annual Re-Registration Fact Sheet and Forms (PDF format)
In April, each student currently enrolled in our school is required to reconfirm their registration for the coming school year. All of the following forms must be submitted along with the $200 re-registration fee:

- Emergency Cards (large and small)
- Registration Record For Child Receiving Care Away From Home
- Authorization For Emergency Medical Treatment
- Fee & Service Agreement
- Child Health Certificate

Schedule Change Request (PDF format)
Requests for changes to your child's part-time schedule must be submitted in writing thirty (30) days in advance and must be accompanied by a $25 schedule change fee. This schedule change fee is non-refundable, and there is no guarantee that the requested space will be available.
Health Certificate (PDF format)
The health certificate must be completed by the child's physician and in the child's file prior to the child's first day. This form must be updated annually and each time the child completes an immunization series as prescribed by the child's physician and the District of Columbia.

Frequently Accessed Policies
The Friendship Children's Center is managed in accordance with Chapter 3 of Title 29 of the District of Columbia Municipal Regulations (DCMR). To ensure consistency and compliance with DCMR, Title 29, Chapter 3, the school maintains a Family Handbook, Employee Handbook, and Curriculum Handbook, which are available to all enrolled families and full-time faculty. In addition, the school has an Operations Manual and Emergency Plan utilized by all management staff and the Board of Directors to ensure efficient and effective operations.
As DCMR, Title 29, Chapter 3, was recently revised, the school is currently undergoing an extensive review process to update all of its policies and procedures in accordance with the new regulations. New "Frequently Accessed Policies" should be posted shortly.
